Immunizations

Florida Department of Health in Martin County


We provide immunizations for school aged children who have Medicaid or are uninsured, and children who are new to the community and have not yet been established with a private provider. We also provide immunizations in response to communicable disease outbreaks in the community.

COVID-19 Vaccinations:

We provide COVID-19 vaccines (Jansen and Jansen, Pfizer or Moderna).

Vaccination Certifications or Exemption Forms:

DH 680 Form (Florida Certification of Immunization):

Contact your private healthcare provider to obtain a DH 680 form, if needed. The health department also provides 680 forms.

DH 681 Form (Religious Exemption from Immunization):

Contact the Florida Department of Health in Martin County to obtain a DH 681 form. Note: Florida Department of Health is the only agency who can provide a DH 681 form.

2022–2023 School Entry Requirements:

Before attending school in Florida (kindergarten through 12th grade), each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:

Seventh Grade Requirements:

An additional dose of the Tdap vaccine is required for students entering the 7th grade. Martin County - School Immunizations:
