We provide immunizations for school aged children or adults who have Medicaid are uninsured or underinsured. Available vaccines include: hepatitis A and B, HPV, influenza (flu), meningococcal, pneumococcal, Tdap (tetanus, diphtheria, pertussis).

Immunizations Certifications or Exemption Forms
DH 680 Form (Florida Certification of Immunization):
Contact your private healthcare provider first to obtain a DH 680 form. If you have recently moved and have not established a healthcare provider, the health department can provide you with a 680 form. Fee: $10, per form

DH 681 Form (Religious Exemption from Immunization):
- Contact the Florida Department of Health in Martin County to obtain a DH 681 form. Florida Department of Health is the only agency who can provide a DH 681 form.

2024–2025 School Entry Requirements
Before attending school in Florida (kindergarten through 12th grade), each child must provide a Florida Certification of Immunization (DH 680 form).
Seventh Grade Requirements
- An additional dose of the Tdap vaccine is required for students entering the 7th grade.
