We provide immunizations for school aged children or adults who have Medicaid are uninsured or underinsured. Available vaccines include: hepatitis A and B, HPV, influenza (flu), meningococcal, pneumococcal, Tdap (tetanus, diphtheria, pertussis).

DH 680 Form (Florida Certification of Immunization):
Contact your private healthcare provider first to obtain a DH 680 form. If you have recently moved and have not established a healthcare provider, the Florida Department of Health in Martin County can provide you with a 680 form. Fee: $10.00 per form.
