Free Immunization Clinic for Students enterting the 7th Grade

By Renay Rouse

July 21, 2015

(Martin County, FL) The Florida Department of Health in Martin County is hosting a free clinic on Friday August 7 from 11:00 am to 2:00 pm at 3441 SE Willoughby Blvd., Stuart, to provide the Tdap (tetanus, diphtheria and pertussis) booster immunization for students entering the 7th grade.

No appointment is needed. Vaccine supply is limited and will be provided on a first come, first served basis. A parent or legal guardian must be present for the child to receive the vaccination. Florida statute requires students who are entering the 7th grade to show proof of the Tdap booster vaccination on the #680 form. If the student already received the Tdap booster, parents should deliver the #680 form to their child’s school before August 24th.

Additional information on immunizations is available at: www.MartinCountyHealth.com
