Birth Certificates

Vital Statistics


Birth Certificates are available for all births in the State of Florida from 1930 to present. Any birth certificates prior to 1930 must be requested from the Bureau of Vital Statistics in Jacksonville.

Birth records under seal by reason of adoption, paternity determination or court order must be obtained from the Florida Bureau of Vital Statistics. For a record under seal write to:

Mail In Requests:

Application for Birth Certificate

A completed application should be submitted along with the appropriate fee. Make check or money order payable to the Florida Department of Health in Martin County. Please do not send cash.

*A legible copy of a valid driver's license, passport, state or military identification card must accompany all requests.*

 
Online/Credit Card Orders:

The Florida Department of Health in Martin County does not accept credit cards for online orders; however, for your convenience, you can process online requests through VitalChek or by phone at 877-550-7330. Additional fees will apply.
