Be Sure to Add Immunizations to Your Back to School List - Don't wait, vaccinate!

ith the start of the new school year just a few weeks away, the Florida Department of Health is urging parents and caregivers to be sure that their student’s immunizations are up to date for school entry.

Proof of certain immunizations, on a DH 680 form (Florida Certificate of Immunization), is required for school entry if a child is entering kindergarten, seventh grade or is moving to Florida from another state or country. The Health Department in Martin County (DOH-Martin) is offering free school age immunizations for children who are uninsured, underinsured or do not have a medical provider. Appointments are available by calling: 772-221-4000, option #3.

A health department team will provide the Tdap vaccine for 7th grade entry at the following outreach events:
